MEDICATIONS

Please do not send any medications with your child to school. 

If medication is needed, please print and fill out needed forms for your child.  Please bring the completed form and medication to Nurse Megan.

Non Prescription Medication Form - This form is for medications that are not prescribed by a physician, such as Tylenol, Motrin and Cough Drops.

Prescription Medication Form - This form is for daily medication given at school that is prescribed by a physician.

Asthma Safety Plan - Use this form if your child has asthma.  When using this form, you do not need to fill out a Prescription Medication Form.

Allergy Emergency Plan - Use this form if your child needs an Epi Pen at school.  When using this form, you do not need to fill out a Prescription Medication Form.

Seizure Safety Plan - Use this form if your child will have Diastat at school.

 **Prescription Medication, Asthma Safety, Allergy Emergency and Seizure Safety Forms
will all need a Physician's signature.**
